In insurance the failure is a **denied/underpaid claim because of a gap** — which harms the client and exposes the agent to an E&O suit. Premortem finds the gaps while they're still fixable. ## Domain inputs → the parent's Process Imagine a claim was denied/underpaid — why? - Limits too low (dwelling underinsured, liability under net worth) → recommend umbrella. - Exclusions bite (flood, earthquake, business-use, water backup) not disclosed/added. - Coverage type wrong (ACV vs replacement cost). - Life changes not reflected (new business, teen driver, home addition, rental). - Document every recommendation the client declined (E&O defense). ## Worked example Client's net worth grew to $2M; still on minimum auto liability, no umbrella. → Premortem: an at-fault accident exceeds limits → personal assets exposed + "why didn't my agent tell me" E&O claim.
